The Digital Journey Planner is an online self-assessment system for GP Practices, we have developed the system with NHS England, to optimise general practice knowledge, understanding and processes, with the aim to improve patient experience.

The DJP is full of useful tools and resources for improvement, centralised into one place to make it simple for practices to make improvements on their digital journey. The DJP makes planning improvements easy by generating action plans based on your answers, giving practices clear and attainable areas for improvement and highlighting the tools and resources available to support with implementation. What modules are available?

The DJP has four modules available to all NHS England practice teams. Practices, PCNs and ICBs can access further modules as they become available in year 2 of the programme, by subscribing to the DJP enhanced licence. Enhanced licence offers PCNs and practices the ability to see their progress and work on improvements together as well as accessing support from the Redmoor Team of primary care specialists via coaching, 1-1, practice or PCN workshops and webinars. The remaining modules are released quarterly to allow practice teams time to work through the assessments and work at their own pace too. We recommend that you work through modules sequentially to allow the building blocks of digital and online services to embed in your practices.

Patient communications

This module is aimed at helping you to educate your patients and practice teams about online services and how you want use them to get the best out of your Practice. Good communications makes it simpler for patients and carers to engage with the practice and understand what service to expect. This module is free for all practice teams to use.

Module Covers:

  • Patient communications introduction: Exploring good communication, consistency of communication and avoiding duplication
  • Messaging: Improving the consistency and speed of communication through effective messaging systems and enhancing digital literacy
  • Website: Exploring design and content to ensure your website is the first place your patients visit with a health need. Aligning your website with your Social Media activity to form part of your communications strategy
  • Social Media: Proactive approach to social media communications add value to your patient communications and build Practice identity online whilst conveying important practice information messages. Embracing the opportunities of closed Facebook groups for peer-to-peer support and patient engagement.

GP online services

This module explores how your Practice enables people to access your services online. Effective use of online services means that Practice Teams spend less time handling enquiries from patients this is particularly the case for simple tasks such as ordering medication, updating contact details, booking and cancelling appointments and accessing care records. This module is available to DJP enhanced licence users only.

Module Covers:

  • Registration: For new patients online and existing patients wishing to use online services
  • Appointment book: Configuration, slots, sessions and patient online
  • Online Medication Ordering: Improving the experience for patients
  • Social Media: Giving patients autonomy
  • Proxy Access: Supporting Care/Nursing Homes to order for patients

Digital inclusion

Digital inclusion is a social issue. An individual's life can be negatively impacted by a lack of digital skills. Practices and primary care networks can use this module to ensure they provide services that are easy and affordable for patients with varying levels of digital competence and literacy. 

In this module, teams will learn how to support patients in getting online and using digital health tools and services to their full potential. This will reduce the likelihood of digital exclusion. This module is available to DJP enhanced licence users only. 

Module Covers:

  • Raising Awareness: Increasing team awareness of digital inclusion and the negative effects of lack of digital skills and health literacy
  • Accessibility: Assisting teams in understanding accessibility functionality in online and digital products
  • Inclusive Communications: NHS services are for everyone. Many UK adults have low levels of literacy, making it difficult for them to read and understand medical content. This section highlights the importance of creating content for and about people that is inclusive and respectful
  • Digital Skills: Primary care teams can work with third sector and community organisations to help raise awareness of NHS online services in the hope they will cascade this to people in the community who may be excluded.

Online Consultations

As part of Digital 'Access', online consultations can increase patient access and streamline practice workflow when used alongside GP Online Services. Using an online triage and consultation system can ensure that appointments are used correctly based on clinical need and importance, and divert patients to alternative treatments. This module will support you to plan your online consultation system, prepare your team and then communicate how best to use the system to your patients. 



Cost

The foundation licence of the DJP contains self assessment and implementation plan for the initial four modules only; Patient Communications, GP Online services, Digital Inclusion and Online consultations.

Enhanced is our annual subscription offer and this will help practices and PCNs to embed the learning as business as usual. It especially supports practices wanting to deliver consistency across PCNs and is popular where PCNs have Digital champions and Digital and Transformation Leads in place. Practice staff can receive 1-1 calls, practice or PCN workshops and catch up on our webinar programme, with topic linked to each of the modules. Annual subscription is 10p per patient, reducing to 8p for the 2nd year.

What is the DJP?

The Digital Journey Planner (DJP) is a web-based self-assessment tool designed to help practices assess their current use and understanding of technology, discover new digital opportunities, solve issues and receive a practice specific implementation plan to help the practice and their patients get the best out of digital technology.

What modules are available on the DJP?

The first module launched is ‘Patient Communication’, this includes help and advice about practice websites, social media and messaging (eg. SMS), these are key areas of focus for improving access. This module is free for all practice teams to use.

GP Online Services includes assessments to support patients registering with practices and for online services to book, rearrange and cancel appointments, order repeat prescriptions online, view medical records online and set up proxy access. This module is available to DJP enhanced licence users only.

Digital inclusion helps practices to provide services that can be easily and affordably accessed by patients with different levels of digital confidence and health literacy. This module is available to DJP enhanced licence users only.

A number of other modules will be released over the next 12 months, all related to digital transformation within general practice, these will available to DJP enhanced licence users only.

What support is available for practices completing the DJP?

The DJP is a practice-based tool. The completion of the self-assessment questionnaire is intended to be relatively straight forward and is designed to be completed by practice staff. However, some practices may choose to bring in additional support to aid the completion followed by implementation of the suggested recommendations.

What user testing was undertaken before the launch of the DJP?

The DJP ran in beta format for 7 months with testing from a number of practices across the country. Redmoor Health continuously engage with practices to understand experience and make improvements to the DJP. They also work with PCNs and ICBs to improve reporting functionality in the Enhanced licence version.

Will CCGs/ICS receive a copy of the implementation plan from the DJP?

DJP is a supportive tool focused at providing practice and practice staff with the knowledge and confidence to introduce and improve the use of digital services. It is not an assurance tool and should not be used in this manner. Commissioners working with practices to support the implementation of resulting plans will need the agreement of the practices to share their plans. Supports needs to be hands-on, working with practices to map out process, identify and resolve problems, training, and communication with staff etc.

What information will be made available at a regional level to help understand what the uptake of the DJP has been?

The national team will have access to who is registered and who has completed the module, which can be made available through the steering group to regional teams. We would encourage you to engage with practices who take up the offer. Details of improvement plans will stay with the practice. However, if you engage with those practices using the tool and they are happy to share their improvement plans then improvement plans can be aggregated holistic view for commissioner and regional teams.

What is the expectation on ICS/ICBs to fund the resulting implementation plan?

The DJP should provide a more in-depth understanding of the digital capability and usage. This should support ICBs to better understand priorities and access Digital Transformation Fund leading to improvements in service delivery and access in primary care.

The resulting implementation plans will support local commissioners to better understand the priorities at PCN/practice level which should feed into the strategic planning at the ICS level.
