Complete the Digital & Transformation Maturity Assessment

  1. Log into your Hub account 
  2. On the Welcome Page 
  3. Select “Start Assessment” 
  4. Answer each question honestly based on your current practice situation 
  5. Complete all the questions (typically takes 15-20 minutes) 
  6. Click “Submit” when finished 

Understand your Digital & Transformation Maturity Assessment

  1. Review your overall maturity score (displayed as a percentage or level)
  2. Examine scores by each transformation theme (pillar) and topic:
    – Communication Foundations
    – Access
    – Patient Journeys
    – Workflow & Efficiencies
    – Workforce & Leadership
    – Demand and Capacity
  3. Identify your strongest and weakest areas
  4. Note any red flags or priority improvement areas
  5. Consider with your PCN share results and identify areas you could work on together

Engage with the Digital & Transformation Network

  1. Login to the Digital and Transformation Network or Open the App on your phone
  2. Browse discussion topics relevant to your interests
  3. Post questions or share experiences with other practices.
  4. Join webinars and virtual events (check the events calendar)
  5. Access shared resources and best practices from the community

How to complete the In-depth Assessment

  1. From your dashboard, select “Assessments” in the top navigation bar
  2. Choose a specific topic area (e.g., social media, telephony)
  3. Complete the detailed questionnaire for that topic
  4. Actions are populated automatically

How to build and access Action Plans

  1. Actions are populated automatically in response to the answers provided in the in-depth assessment questions.
  2. Work on your actions immediately by clicking the pick pop-up box in the top right corner or you navigate to them at a later point by selecting actions from the navigation bar at the top.

How to complete an action

  1. Navigate to the action tab
  2. Select the topic from the action plan summary
  3. Select the action
    Tip: it is best to work on power actions where you see this symbol as these actions have multiple impacts)
    Tip: Consider the priority allocated to each action when you are panning how you are going to complete them
  4. Click get started and you will be taken to the specific section of the knowledge base where you will find out why it is important and what you need to do.
    Tip: You can allocate a task to someone in your team by clicking on Add Person and allocating the task to them.
    Once you have made the change complete the action by clicking complete action.

Use the Digital Maturity Index to baseline, review and track your digital progress

  1. The DMI can be found in the tab in the top navigation bar.
  2. You can navigate within the DMI using the tabs at the top
  3. You can select the relevant ICB, PCN or Practice from the filters on the right-hand side.
  4. You can identify most recent performance and trends over time which may inform your next steps and identify priorities.

Navigating the Knowledge Base

  1. You can access the entire knowledge base at any time from the top navigation bar.
  2. You can also search for contents from the knowledge base using the search bar and selecting the knowledge base filter.

The Hub is built around six transformation themes to address the specific needs of primary care:  

Communication Foundations

This pillar evaluates how practices communicate with patients through digital channels, ensuring clear, inclusive, and effective engagement. It aligns with the NHS’s commitment to digital inclusion, ensuring that no patient is left behind.

Access

This pillar assesses the ease with which patients interact with services—whether through websites, telephony, care navigation, or self-referral options. It directly supports MGPA's focus on streamlining patient access to ensure a smooth digital experience.

Patient Journeys

Understanding and optimising digital patient journeys is critical. This pillar examines how well practices manage digital interactions like appointment bookings, test result access, repeat prescriptions, and patient record access, helping patients become more active participants in their care.

Workflows & Efficiencies

Digital transformation should make practices more efficient, not more complex. This pillar focuses on improving the use of electronic patient records (EPRs), remote monitoring, and front-office tasks, ensuring that digital tools create a smoother, more productive working environment.

Workforce & Leadership

Transformative change requires strong leadership and a workforce that’s ready for digital evolution. This pillar evaluates how practices prepare their teams through training and how leaders drive transformation, ensuring a supportive environment for digital change.

Demand & Capacity

As the NHS emphasises data-driven planning, this pillar helps practices leverage data to manage patient demand, adjust resources, and predict needs more effectively. It ensures that practices can be more responsive, using data to inform decision-making and improve service delivery.
