At Redmoor Health, we understand the critical role that effective communication plays in patient care. Our Communication Foundations project is designed to enhance the way Primary Care Networks (PCNs) engage with their patients, ensuring clear, consistent, and inclusive communication.

Why it's important

Effective communication is the cornerstone of successful patient care. It improves patient understanding, engagement, and satisfaction while reducing health inequalities and enhancing overall service delivery. In today’s digital age, mastering communication across various platforms and understanding patient preferences is crucial for PCNs. 

What the project is about

The Communication Foundations project aims to build robust communication strategies that cater to diverse patient populations. By focusing on patient communications, engagement, digital inclusion, and change management, we help PCNs create a cohesive communication framework that meets the needs of all patients. 


  • Patient communications: Developing effective ways of communicating with different cohorts of patients, ensuring messages are clear and accessible.
  • Patient engagement: Enhancing Patient Participation Group (PPG) involvement and dialogue, establishing feedback mechanisms, and fostering better patient-provider relationships.
  • Digital inclusion: Identifying patients who may be digitally excluded and finding the best ways to overcome or manage these barriers to ensure no one is left behind.

  • Patient engagement strategies: Developing effective strategies to engage patients in their care and service development. 
  • Patient activation: Empowering patients to take an active role in their health and care.

  • One system approach: Ensuring a unified approach across the entire healthcare system. 
  • Digital ambassadors: Training staff to become champions of digital communication and inclusion.

  • Change foundations: Providing tools and guidance on managing change for both patients and staff to facilitate smooth transitions in communication practices.
